Request Biblical Counseling
Thank you for taking this step. Below you’ll find a short form to help us understand your situation and connect you with the right counselor. Once submitted, you’ll be assigned a counselor who will reach out through HopeTrack with a more detailed intake questionnaire. After that’s completed and returned, we’ll schedule your first session.
Before you begin, a few things to know: We require that your pastor be made aware that you’re receiving counseling through us, and that he consents to it — we ask for his contact information below so we can notify him. We will not share any details of what you discuss in counseling without your permission. We also ask that you describe honestly what you’re seeking help with, so we can match you with a counselor equipped for your situation.
If you are in crisis or need immediate help, please do not wait for this form to be processed. Call or text 988 (Suicide & Crisis Lifeline) for immediate support, or call 911 if you are in danger.
After you submit this form, here’s what happens next:
– We’ll review your request and assign you a counselor.
– They’ll contact you through HopeTrack and send a more detailed Data Inventory to gather what’s needed to begin counseling.
– Once you complete and return that, they’ll reach out to schedule your first session.
